The significance of the tumor microenvironment for cancer growth, progression, and metastasis is well recognized. Stromal cells and their extracellular matrix (ECM), specifically, are major factors determining cancer pathophysiology. For instance, in lung adenocarcinoma, the stroma component of the cancer to a great extent establishes the clinical manifestation of the disease and bears significant prognostic information. Cancer-associated fibroblasts (CAF) in the stroma can modify cancer epithelial cell phenotype by direct cell-cell contacts, the secretion of soluble factors, or the modification of the ECM. Such crosstalk between CAF and epithelial cancer cells has been implicated in the development of therapy resistance. In this sense, creating physiologically relevant ex vivo models to study CAF-cancer cell interactions holds great potential for cancer therapy.
